2016 Political Candidate Questionnaire

On a separate sheet of paper, please provide as much information as you feel necessary to respond to the questions listed not to exceed one 8×10 ½” paper.  Please format your responses under the heading of each of the questions shown. Return the completed questionnaire to the Hawaiian Affairs Caucus to e-mail: hawaiian.affairs.caucus@gmail.com by not later than July 20, 2016, 4:00 p.m. Responses received after that date will not be considered. Posting of information on the website will be in the order of information received, thus early response is encouraged.
